General Practice Pharmacy Services
- Help patients and carers manage prescribed medicines and make shared decisions about their Long Term Conditions (LTCs).
- Increase patient knowledge and understanding about their treatments, adherence to medications and consequently have an impact on their health.
- Promote multidisciplinary working and inter-professional learning within General Practice.
- Link the role of the Pharmacist to gains in the productivity in General Practice e.g. enhancement of GP Practice KPIs, QOF, LES and DES agenda.
- Support integration with other healthcare providers and provide appropriate signposting and referrals.
- Help with reductions in medicines-related hospital admissions and improve quality of life for patients.
- Enable an integrated primary care network between GPs, Community Pharmacists and other allied healthcare professionals.
